A consent form will be sent once the first appointment is scheduled. It reviews the policies and practices for Mind Rising. Please read it carefully and contact me with any questions.

RELEASE OF INFORMATION

The Release of Information form can be downloaded here if you would like me to coordinate care with any professionals you have worked with now or in the past.

GOOD FAITH ESTIMATE

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.
